11.4 Clarifying Goals and Roles for Both Systems

Good public policy provides a framework for people working in public systems. It defines why, what, how much, when, by whom and how the results will be assessed and reported. This study has found that current policies within both systems may not be meeting these standards.

This suggested policy direction has two components; (1) providing clearer direction and outcomes and (2) clarifying the role of public health in working with schools.

Providing Clear Direction and Outcomes

This study has found that the long-term policy goals for both systems relative to sexual health, STD and to a lesser extent, HIV, are not clearly understood by people working in both systems. Written action plans are not often in place. Curriculum documents do not indicate the time required to reach the required learning outcomes. Guidelines on the role of public health nurses are not clearly defined in most cases.

We recommend that research be done to clarify the minimum learning outcomes and time that should be required of sexuality education programs and the minimum levels of service and support related to other determinants of health are adequate, including adolescent sexual health services, peer and parent involvement and a safe, healthy school environment. These outcomes/determinants guidelines should be disseminated widely.

Clarifying the Role of Public Health

This study has found that the levels of direct public health services to schools are declining, that the roles of public health professionals in working with schools are not clear, that the public health system is in a period of transformation, that public health policies on adolescent health need development and that professionals in the system are not fully qualified to work with youth or schools.

We recommend that public health authorities develop policies on adolescent health and the capacity of schools to reach youth. We further recommend that research be undertaken on the cost-effectiveness/cost-benefits of public health nurses working closely with schools and that comparisons be done with other countries in regards to this deployment of public health nurses.
